Rental Terms and Conditions
I understand that rented equipment is the property of the organization and is not to be sold, given, borrowed, or used by any other persons.
I understand that I am not to perform any repairs to the equipment and agree to notify the organization of any malfunctions, maintenance needs or repairs. I understand that during the rental term, equipment will be repaired or replaced at no extra cost, unless, in the sole determination of the organization, the damage is caused by the user's misuse or neglect, nor does it apply to any product which has been repaired or disassembled.
I agree to keep the equipment at the address provided on file during the rental term and agree to notify the organization of any changes in residence, contact information, or status.
I hereby confirm and am satisfied that the necessary setup and equipment safety and operation checks were performed by the organization at the time the equipment and/or services were provided. I received both an equipment demonstration and verbal instructions on the use, maintenance, and cleaning of the equipment. I further received and reviewed additional printed education material and safety precautions. Lastly, I have been provided the name and telephone number to contact should I have any questions regarding its care and operation or in the event that equipment should require servicing.
I acknowledge acceptance of this equipment in good working order, operating properly and adjusted appropriately for my use.
I hereby release and hold harmless the organization, its owner, agents, employees, heirs and assigns for any liability whatsoever as a result of my failure or negligence to ensure the proper operating procedures and instructions are followed as outlined and provided to me.
Unless purchased or included, I am responsible for delivery and return of equipment.
I agree to forfeit all or a portion of my deposit if the equipment or any parts thereof are lost, stolen, shows abnormal wear and tear or shows evidence of unauthorized repair or dis-assembly.
